How often should I review my condition?
Most stable conditions are reviewed every 3–6 months. We'll see you sooner after medication changes, flare-ups, or new symptoms, and complete a comprehensive annual review with updated tests.
What should I bring to my appointment?
Bring your ID, medical aid details, medication list (or photos of labels), home readings (e.g., blood pressure or glucose logs), and recent test results or hospital letters. Devices like a glucometer or BP cuff are helpful.
How do you coordinate with specialists and allied health?
We send structured referral letters, share relevant results, and agree a clear care plan. Feedback from specialists and allied clinicians is tracked, and we integrate their recommendations into your ongoing management.
